In a natural monthly cycle, a woman's body releases enough follicle-stimulating hormone (FSH) to mature only a single egg. During an In Vitro Fertilization (IVF) cycle, injectable gonadotropins provide supra-physiological hormone levels to encourage multiple resting follicles to mature together.

Individual response to fertility medications varies based on patient age, baseline FSH and Anti-Müllerian Hormone (AMH) levels, antral follicle count (AFC), and ovarian volume. Common stimulation medications prescribed at VCRM include:

Recombinant FSH (rFSH)

Gonal-F and Follistim contain pure recombinant follicle-stimulating hormone to drive follicular growth with high batch consistency.

Highly Purified Menotropins

Menopur provides an equal 1:1 ratio of FSH and LH activity (derived from hCG) to support thecal cell function and oocyte maturation.

Administration Protocol

Injections are taken subcutaneously once daily (usually in the evening) or divided twice daily for approximately 10 to 14 days under regular clinic monitoring.

Preventing Premature Ovulation

Ganirelix Acetate & Ovulation Control

As follicles grow and secrete high amounts of estrogen, the pituitary gland naturally wants to trigger an LH surge, which would cause premature egg release before retrieval. To prevent this, an antagonist or agonist is introduced:

Ganirelix Acetate (GnRH Antagonist)

Ganirelix immediately blocks GnRH receptors in the pituitary. It is initiated mid-stimulation once the lead follicle reaches 12–14 mm in diameter. Ganirelix is administered every evening for approximately 3–5 days until the trigger injection. Taking this injection at the same time each night is vital to maintain receptor blockade.

Lupron (GnRH Agonist) Down-Regulation

For certain clinical indications (such as endometriosis or specific micro-flare protocols), Lupron is used prior to or alongside stimulation to suppress natural hormonal surges.

Baby Aspirin (81 mg)

Dostinex / Parlodel

Folic Acid & Prenatals

All patients should take prenatal vitamins with folic acid. Proven to reduce neural tube defects (such as spina bifida) during the first 50–70 days after conception.

Once leading follicles reach optimal maturity, you will receive precise instructions to administer your human chorionic gonadotropin (hCG) or Lupron trigger shot. This trigger finalizes the eggs' meiotic division.

The egg retrieval procedure is scheduled exactly 35 hours following the trigger injection.

What are the common side effects of gonadotropin injections?

Most women experience mild pelvic fullness, abdominal bloating, breast tenderness, and occasional mood changes due to elevated estrogen levels. Local injection site redness or mild bruising is also common.

What should I do if I accidentally miss an injection?

Contact our clinical nursing team immediately. Do not double up on your dose unless explicitly instructed by Dr. Sharara. Timing consistency, particularly with Ganirelix, is essential to cycle success.

Why do older patients or women with high FSH require higher doses?

As ovarian reserve declines, follicular granulosa cells have fewer FSH receptors. Higher doses of gonadotropins (or protocols like DuoStim) are utilized to encourage optimal follicular recruitment.
